This SDXL model enables a shocker handsign. Works for both photos and paintings. Trigger word is 'shockr', but using the phrase 'shockr hand sign' is also helpful in coaxing a good shocker...

As hands are tricky, may need to negative prompt for finger count or disfigured hands.
