This pose is akin to the iconic Black Jack pose.

I didn't specifically focus on the operating room atmosphere. I created this because it was surprisingly difficult to capture the character's palm facing inward.

The dataset contains many images of characters wearing surgical gowns, so the fingers tend to be thick. For bare hands, I recommend applying a detailer.
