v1.1 update: in the interest of greater subject matter versatility, I sacrificed a little of the charming, sloppy drawing style of the initial version. But it still retains a very hand-drawn old-school comics look.

I can add to or correct an existing thought bubble, but is there a way to inpaint a new thought bubble. I can't seem to make that work.
I haven't tried, but personally, if I wanted to be sure to get a specific text box or speech bubble, I'd create the shapes and text in Photoshop (actually, I use Affinity Photo/Designer nearly 100% of the time).
