Hard to consistently get my preferred look for half-orcs, so I trained up a lora. The reference model is 'Blue' who might be only about a quarter orcish. Tusks are decent, but in my testing you want the keywords "orc, tusks" and to specify skin tone. Clothes will tend to default to a blue beret and armor, or fantasy medieval tavern girl, so if that's not desirable, put them as negative prompts.
